New Patient Registration

If you live within our practice area (North Hykeham, South Hykeham, Witham St Hughs, Thorpe on the Hill, Whisby, Eagle, Harby) then please visit our surgery and a receptionist will explain the registration process to you.

Registration forms and medical questionnaires are avialable to collect from the Practice.

Choice of Doctor

Patients are free to consult with any doctor in the practice but may express a preference when arranging an appointment. We respectfully remind patients that by requesting a specific doctor this may reduce the choice of appointment slots available.

Medical treatment is available from the date of registration. Please contact reception for further information.

Permission Forms

Anyone who is 16 and over will need a permission form filling out if you would like someone else to have access to your records. Please click on link below to print this out and the return to reception, or alternately come into the surgery and collect a copy here.

A Patient Guide to GP Services

These fact sheets have been written to explain the role of UK health services, the National Health Service (NHS), to newly-arrived individuals seeking asylum. They cover issues such as the role of GPs, their function as gatekeepers to the health services, how to register and how to access emergency services.

Special care has been taken to ensure that information is given in clear language, and the content and style has been tested with user groups. Open the leaflets in one of the following languages

Temporary Registration

If you are ill while away from home or if you are not registered with a doctor but need to see one you can register as a temporary resident for up to three months. 

This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary resident again or permanently register with the Practice.

To register as a temporary resident simply contact the local practice you wish to use. Practices do not have to accept you as a temporary resident although they do have an obligation to offer emergency treatment. You cannot register as a temporary resident at a practice in the town or area where you are already registered.
