Welcome to Crossroads Medical Practice
With patient’s needs at the heart of everything we do, our website has been designed to make it easy for you to gain instant access to the information you need. As well as specific practice details such as opening hours and how to register, you’ll find a wealth of useful pages covering a wide range of health issues along with links to other relevant medical organisations.
Information for patients registering with Crossroads Medical Practice
Patients registered at Crossroads Medical Practice will be transferred over to Richmond Medical Centre from 1 April 2020.
The move comes after the GP partners at Crossroads left the practice in November 2019. Crossroads is currently being temporarily run by LCHS. The Partners of Richmond Medical Centre are working with the Clinical Commissioning Group and LCHS to ensure any disruption to services will be kept to a minimum. Patients should be confident that from the 1st of April there will be a wide range of appointments available both at the Richmond premises on Moor Lane and the current Crossroads premises on Lincoln Road. The latter will become the ‘Village branch’ of Richmond Medical Centre.
As a larger Practice, with more clinical rooms and staff, the quality and range of primary care services available to patients will improve. There will be a wider variety of clinicians and appointment times.
Please be aware that from the 1st April 2019 you will no longer be able to collect your repeat prescription from the surgery. Please contact the surgery to set up for a pharmacy of your choice who will be open for you to collect your prescription on an evening or weekend. If you are housebound please contact the pharmacy to arrange delivery.
Get Well, Keep Well
Of course we’re not just here for when you are unwell. Our team of healthcare professionals and back-up staff offer a number of clinics and services to promote good health and wellbeing whatever your medical condition.
If you would like to register with us, you can either collect a registration pack from reception or follow the registration form link below this paragraph. Please complete the forms as fully as possible and give as much information as you can, this will help us to ensure that we can process your request promptly. Once the forms are complete please return them to the surgery.
We hope you enjoy having a look around the site and familiarising yourself with some of the online features such as ordering a repeat prescription. Whatever your thoughts, be sure to let us know via our feedback function. Comments and suggestions are always a great way of helping us continue to enhance the way we look after you.
Disabled parking and toilet facilities are available. Access to all patient areas can be achieved without stairs or steps. Non-patient visitors can also access our meeting room via the stairlift.
The surgery shares a common car park with shops and other services at the village centre. When attending the surgery please allow extra time to park. There are 9 spaces for disabled parking.
We have a Patient Participation Group (PPG) details of which can be found on the Patient Group Page. (also shown on the right panel on the Orange Button)
