Changes at Crossroads Medical Practice November 2019
All households with patients registered at Crossroads should have received a letter explaining the changes currently affecting the surgery. Patients should continue to utilise the services at the sugery as usual. The letter advises those with any concerns to contact PALS.

FLU VACCINATION 2019/2020
If you are eligible for a seasonal flu vaccination but you have not received your free injection this winter please contact the surgery to book an appointment.
Accessing a medical appointment outside of our opening hours
Crossroads Medical Practice is pleased to be able to offer our patients access to routine GP appointments at evenings and weekends.
The new service is part of an extended access service which has been commissioned by NHS Lincolnshire West Clinical Commissioning Group.
The appointments will also be available on bank holidays and across the Easter, Christmas and New Year periods
Extended hours appointments are for pre-bookable non-urgent appointments.
For urgent matters at evenings and weekends, patients will continue to access the GP Out of Hours Services at Lincoln County Hospital via NHS 111 or the Minor Injuries Unit at John Coupland Hospital.
The service is being delivered by a collaboration of GP practices in the area. If a patient wishes to attend an extended hours appointment, they may need to travel to another local GP practice or a central GP practice hub and may not necessarily see their own GP.
Ultimately, this is about allowing a patient to see a GP or practice nurse at a time that is convenient to them.
You can now book the appointments outside of our normal hours and at weekends.

The General Data Protection Regulation (GDPR) is a new law that determines how your personal information is processed and kept safe, and the legal rights that you have in relation to your own data.

Named Allocated Gp
All patients will have been allocated a named accountable GP. You can ask at reception or at your next appointment to find out which GP has been allocated to you. If you would prefer a different named GP, let us know and we will do our best to accommodate your choice. You can continue to see any of the GPs in the practice, irrespective of which GP you have been allocated.
